Bradford and Beyond: physical health template takes off in primary care

A physical health template implemented into SystmOne is revolutionising care for patients with serious mental illness.

Originally developed in Bradford, the template has been picked up nationally, and is now being used by 610 organisations in 59 different clinical commissioning groups across the country.

Research has shown that patients with serious mental illness die up to 20 years earlier than the general population, mainly from preventable, physical causes.

The Mental Health Physical Review template helps clinicians in primary care to carry out an annual physical health check, and also calculates the risk of a patient contracting conditions such as cardiovascular disease and diabetes. To date, more than 2,600 health checks have been completed.

The template was piloted by six practices in Bradford, and then rolled out regionally with the support of the Yorkshire and Humber Academic Health Science Network.

This case study describes the evolution of the template, and talks to Kate Dale, the former mental health nurse and creator of the template, about the project and its impact.
