West Yorkshire ICB goes live with continuing healthcare platform

- It also supports monthly reporting requirements set out by NHSE
West Yorkshire ICB has gone live with a digital continuing healthcare (CHC) platform, at Bradford District and Craven Health and Care Partnership (HCP) to help ease the transition from children and young people’s to adult care.
CHC assessment is a process that helps determine if people with significant ongoing health needs are eligible for care and support through NHS CHC.
The IEG4 platform was launched on 5 February 2025 with the aim of improving the All Age Continuing Care (AACC) process, which lays out the transition from children’s and young people’s CHC to adult CHC procedures, determining eligibility for funding via assessment.

The platform integrates multiple processes into a single digital system to support staff in managing the entire assessment and referral process end-to-end and enhance operational capabilities.
It is also capable of mapping data fields to meet the required fields in the Patient Level Data Set (PLDS).
As required by NHS England, the ICB will be able to generate an extract of the data required for the monthly reporting requirement. Eventually, all statutory reporting will be done through the PLDS.
The platform also supports the digitisation and automation of referral forms for patients.
A portal provides users of the platform with real-time updates on progress and offers access to administrative and payment tools where necessary.

“In addition, positive feedback from their user-base on IEG4’s client-focused, partnership-driven approach further affirmed our decision, highlighting their commitment to client collaboration and impactful solutions.”
The IEG4 CHC platform was also selected by Somerset ICB, which went live with the platform in October 2024 to help ease the transition from children and young people’s care to adult care.
